Chronic Care Management

With more than one chronic medical condition, a recent illness, and/or a new diagnosis, the needs and struggles patients can have in the home on a day-to-day basis are very real. When you add in healthcare disparities and problems accessing care, literally each day can be a challenge. 

That’s why we are offering this program. Our purpose is to provide our patients with knowledge, support and resources to manage chronic conditions and improve their quality of life.

How it Works

To get started, just ask your provider for a referral for chronic care management.

As a participant in HOA’s Chronic Care Management Services, you will receive:

  • Regular phone calls with the chronic care manager to:
      • work to achieve your goals for health and well-being
      • help coordinate your care
      • answer questions you may have about your health, chronic conditions, and/or how to navigate the health system
  • A printed copy of a comprehensive care-plan from our practice to help you understand how to manage your chronic conditions so that you can be as healthy as possible
  • Access the chronic care manager for questions by calling 315-472-7504, ext. 1188 Monday – Friday from 8 a.m. – 4:30 p.m.
      • If you need to reach your oncologist/hematologist please call 315-472-7504. There is always a provider on call outside of regular office hours.
      • In the case of an emergency call 911.

In order to effectively coordinate your care among various medical providers, the chronic care manager will record information in your electronic medical record and may share information with other providers and specialists. As with all your care at HOA, this reporting will be completed with all precautions to insure the privacy and security of your health information.

What is the Cost?

HOA will bill your insurance, Medicare and/or Medicaid for this service. Therefore, any co-pay or cost sharing that generally applies to your office visit may apply to this service, as well.

NOTE: Only one of your healthcare providers can bill for this service. Please let your HOA team know If you already receive chronic care management services from another provider

About Your Chronic Care Managers

Janet Suchewski and Carol Christiansen work out of the HOA Brittonfield office location, and are available Monday-Friday, 8:00 a.m. - 4:30 p.m.
